The form needs a browser (it uses Cloudflare Turnstile). Fields: business name, address, phone, website, social links, description, hours, breeds, registration, health tests on parents, akc breeder of merit, breed club member, visits by appointment, written health guarantee, takes dogs back for life, waitlist, deposit, puppies from, getting your puppy home, and the submitter's name, email and relationship to the business.
